5-Year State Plan


Councils on Developmental Disabilities in every state must develop a 5-Year State Plan, with annual updates and modifications. Prior to the state plan development and implementation the MSCDD conducts a statewide needs assessment. Services and supports must be based on needs identified by people with developmental disabilities, parents and family members, service providers and community input. The period of our present State Plan is October 1, 2001 through September 30, 2006. We are preparing for the next 5-Year State Plan (October 1, 2006 through September 30, 2011).

The Council on Developmental Disabilities bases our funding priorities on needs expressed by persons with developmental disabilities/disabilities and their families. We need your immediate and continual input throughout our 5-Year State Plan.

FY 06 State Plan - Planning Alternative Tomorrows with Hope (PATH)

The starting point of our state planning process began with a meeting during May 2005 when 30 decision makers representing persons with disabilities, parents, state agencies and private non-profit groups who gathered to share their vision for the future, addressing state needs. From this meeting a working document called Planning Alternative Tomorrows with Hope (PATH) was produced and is located in PDF. It offers a detailed description of needs expressed by the people represented. The PATH process, including the document, is designed to develop a visionary plan addressing needs through specific action steps. The MSCDD State Plan Development Committee welcomes your input. Please respond to the parts of the PATH you feel are most needed and/or add additional information concerning your vision, your needs and your desired outcomes.
